・House visits

Home visits involve regular, planned medical care, once every week or two, for patients who are in the terminal stage of cancer, or who have declining physical functions due to senility or dementia, and are therefore unable to visit the hospital regularly.

This is done by the doctor and clinic staff going to the patient's home or facility where they are resting.

Before a home visit, a care service provider meeting is held in coordination with the nursing staff, visiting nurses, care managers, community comprehensive support centers, home care providers, etc. who are assisting the patient to determine the specific details.

We will review all aspects of support, including not only medical care but also nursing care and daily living.

In the event of an emergency, we will make emergency visits or house calls and, depending on the situation, will even arrange for hospitalization, providing 24-hour support for home care.

A contract must be signed before any medical visits can begin.
